If you need a copy of your accident report that occurred within the boundaries of Washington County, please fill out the form below. If you have your case number, please provide it in the ‘provide additional details’ box. Please allow up to 5 business days for processing of your accident report. This is NOT to file an accident report, rather for requesting an existing report.
Accident reports can also be requested via USPS by mailing to:

Request Process:
- Enter the necessary information into the form below. All required (*) fields must be completed.
- Select the delivery method desired: email, fax or United States Postal Services.
- Your accident report will arrive via your selected delivery method.
